Mechanism

       Imbalance between

·                                         ·    Acid production ( Aggressive factor)

·                                         ·    Mucus production ( Defense factor )

Etiology

·                                         ·   Non steroid anti-inflammatory drugs

·                                         ·    Helicobacter pylori infection

·                                         ·    Other factor : alcohol, smoking, stress, steroid, chronic illness

Clinical Signs

·                                         ·    Epigastric pain

            -       Burning before or after meal about 90mn

            -       Relief of pain with antacid

            -       Tenderness

·                                        ·    Dyspepsia, anorexia, asthenia

·                                        ·    Nausea and vomiting

·                                        ·    Hematemesis or melena

Paraclinic

·                                        ·     Anemia ( Iron deficiency )

·                                        ·    Helicobacter test

·                                        ·    Radio barium or fibroscopic with biopsy

Differential Diagnosis

·                                        ·    Gastro-esophageal reflux

·                                        ·    Hiatal hernia

·                                        ·    Gastrinoma

·                                        ·    Ischemic heart disease

Management

1        1- Avoidance of alcohol, smoking, coffee, NSAIDs
  2-   If gastralgia with antacid : Phosphalugel® or Maalox®
  3-  If gastric hemorrhage

-       Adona® or vitamin K

-       Use ice-compresses

-       If emergency should be take blood transfusion

4.          4-  Helicobacter pylori eradication

-       H2 inhibitor ( Ranitidine® )

-       Proton pump inhibitor ( Omeprazole® )

-       Sucralfate®
